Director Buddy Cooper was a lawyer when he made the original movie, but he'd always wanted to be a filmmaker. He said he read in Variety that 30 percent of all tickets sold at that time were for horror pictures so that's why he decided to make a horror movie. He was planning to make a sequel but the first film lost money due to his refusing to cut enough scenes to earn an R instead of an X. Regardless, he said, "Making 'The Mutilator' was the most fun, most rewarding thing I have ever done," and he's wanted to make another movie ever since. This sequel finally gave him the opportunity.
Casting calls posted on the Backstage website in January 2022 revealed that the roles of Liz and Iza would require actresses willing to perform topless in panties for one scene.
The first poster was revealed in December 2022. It featured actress Eva Hamilton, who plays Olivia, in her underwear bound to a piling on a pier by an ocean.
The film returned to the infamous condo on the beach from The Mutilator for one scene. When the cast and crew showed up to begin filming, they learned that the current occupant of the condo is a big fan of the original film and brought out her framed Mutilator poster. They invited her to stay and watch the day's filming, which she happily accepted.
