Dr. Shutt begins to do surgery again although his future in psychiatry is uncertain. A "suicide doctor" tries to get the hospital transplant committee to use organs that were not harvested properly, and promises one to Austin's patient without her consent. Sarin gas is released in a bank threatening many lives including Hancock's, who risks his own to save others. Wilkes loses it after the incident on the heels of the previous shooting at his son's school, leading him to take a drastic step. Diane returns to work after her maternity leave.

Dr. Waters has a fever and gets a visit from Alan, the Chicago Hope lawyer who was shot and killed. Camille returns and her and Aaron rekindle their romance, Dr. Geiger and Dr. Nyland also pay a visit. A man has multiple surgeries while his disabled wife waits and Dr. McNeil gets an unexpected reward.

Jeffrey Geiger returns having bought controlling interest in the hospital and he fires Austin, Kronk, Yeats, Catera, and Hancock. They decide to pursue their options. Aaron consults with the new pediatric surgeon, Gina Simon about a new born baby. And a young surgeon who was hired by Austin before her dismissal arrives and Geiger puts her to work.
